Agencies that received a Community Impact Grant for 2023 are required to complete a Community Grant Final Report and Community Impact Grant Outcome Report
- If your agency received one or more Community Impact Grants from the 2023 Community Impact Fund last year, the Community Grant Final Grant Report is required.
- The report comes in two parts, The Program/Financial Report and the Outcomes Report.
Reports must be filled out for each Grant that you were awarded.
Example: If you received a Grant in both Bay and Gulf counties, a complete Grant report for each county will need to be completed.
If funds were granted for more than one program, a complete report for each program will need to be completed.
Grant reports are due no later than 4pm on Friday June 28, 2024.
**Please note that failure to complete a 2023 grant report as requested may result in a reimbursement of grant funds to United Way of NWFL and a denial of future funding.
